AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS

APPLICANT REQUESTS THAT:

His SF 88, Report of Medical Examination, dated 16 Aug 68, be 
removed from his records.  


APPLICANT CONTENDS THAT:

The following items on his SF 88 are incorrect:

	Item 44, Dental:  He only had two wisdom teeth extracted 
while on active duty.

	Item 51, Height:  He is 73.5 inches and not 70 inches.

     Item 52, Weight:  He was 185 pounds and not 155 pounds when 
he separated.

     Item 54, Eye Color:  His eye color is hazel and not blue.   

     Item 61, Near Vision:  He was diagnosed with astigmatism 
while he was in the military which resulted in the Air Force 
providing him with reading glasses.

     Item 71, Audiometer:  He was never given an exit exam for 
hearing.

The applicant provides no rationale as to why the Board should 
consider it in the interest of justice to consider his untimely 
application.

STATEMENT OF FACTS:

On 30 Mar 65, the applicant entered the Regular Air Force and on 
18 Oct 68, he was released from active duty.


AIR FORCE EVALUATION:

AFMOA/SGH recommends removal of page two of the applicant’s SF 88, 
dated 16 Aug 68.  They recommend no corrections be made to Item 
44, Dental, page one as there is no documentation to support that 
he had two lower wisdom teeth removed while stationed at South 
Ruislip Air Base, England.  The second page of the applicant’s SF 
88 does not include a name or SSN indicating that it is likely a 
misfiled document that belongs to another member’s medical record 
as it contains the incorrect height, eye color, hearing and vision 
information.  The provided copies of the applicant’s official 
passport dated 8 Aug 68 and statement of personal history reflect 
the applicant’s height at 6 feet and 2 inches and his eye color to 
be hazel.  The SF 88 does not reflect any missing teeth.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

A copy of the Air Force evaluation was forwarded to the applicant 
on 25 Aug 14 for review and comment within 30 days (Exhibit D).  
As of this date, no response has been received by this office.


THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by existing 
law or regulations.

2.  The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file.

3.  Insufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice to warrant 
removing the applicant’s SF 88 dated 16 Aug 68, from his record.  
We took notice of the applicant’s complete submission in judging 
the merits of this case; however, we do not find the evidence 
presented sufficient to convince us that the SF 88 should be 
removed.  The applicant also contends that Item 44, Dental is 
incorrect; however, the applicant has not provided sufficient 
evidence to support his contentions.  Should the applicant provide 
evidence, such as dental records to substantiate the SF 88 is 
incorrect, we may be willing to reconsider his application.  In 
view of the above and in the absence of evidence to the contrary, 
we find no basis to grant the relief sought in this portion of his 
application.

4.  Sufficient relevant evidence has been presented to demonstrate 
the existence of an error or injustice to warrant removal of the 
applicant’s SF 88 dated 16 Aug 88, page 2 which (includes Items 
51, 52, 54, 61, and 71).  Therefore, we agree with the 
recommendation of the Air Force OPR and adopt the rationale 
expressed as the basis for our decision that the applicant has 
been the victim of either an error or an injustice.  Accordingly, 
we recommend the applicant's records be corrected to the extent 
indicated below.


THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air Force 
relating to the APPLICANT be corrected to show that his SF 88, 
Report of Medical Examination, dated 16 Aug 68, page two, be 
removed from his records.
